Rates will again be based on the tonnage that the truck hauls.  Please review the document below and provide your signature and date to show you have read this information.

STEP 4: Requirements for Tarps & EZ Pass for Thruway Jobs

Please review these requirements and make sure your truck(s) are in compliance before hauling any blacktop for us.  Provide your signature and date below to show that you understand these requirements.

STEP 5: Timesheet Procedures

Please read the document below about the procedures and policies for properly filling out and submitting our daily time sheets (sample time sheet included). Provide your signature and date below to show that you have read this information.

STEP 6: Safety Requirements & Specifications

Please read the document below about our company safety policies as well as the NYSDOT Equipment Safety spec. Provide your signature and date below to show that you have read this information.

STEP 7: Insurance Requirements

Please provide this information (document below) to your insurance carrier and then send us a Certificate of Insurance so we have a record that the proper coverage is in place. Also, fill out the W-9 form (document below) and return it to us.
